CTE Procedure for PD time off

If CTE teachers plan to attend any PD, We need the following….

Please submit forms to Emily and Myself 35 days prior to the PD, Late forms will not be accepted/approved. We will make sure that the forms are approved by your principal prior to creating any PO’s

Overnight Field Trip

     CTE Procedures

Overnight Field Trip w/Students 

If  CTE teachers plan to take students on overnight field trips, We need the following…

All Overnight field trips require the Overnight Field Trip Packet to be completed. 

  • Fill out an overnight trip packet. (Set up a meeting with Jamie West if you need assistance filling out packet) 

    • Flyer/agenda for trip 

    • Quotes for Hotel, transportation and flights if applicable

    • If you are using any club funds, Email Veronica Fierro to get your current club fund balance. 

  • Complete REQ’s for all associated costs and submit on the CTE REQ submission form. Please include a flyer with cost listed or quote  

  • Fill out leave requests for Yourself and any other MUSD staff member attending the trip.    *Include Activity registration info and Agenda  

  • Submit time off in Both OKTA and Frontline (Sub Request)  

  • Health Office Notifications 

    • Email a list of students who you anticipated will attend the trip on the day you hand in your packet(95 days prior).

    • Once the trip is Board approved -pass out permission slips.  Email the list of student that received permission slips 

    • The Completed permission slips need to be turned in to the health office at least 30 School days before the trip.

Day Field Trip

     CTE Procedures 

Day Field Trip w/Students

If CTE teachers plan to take students on a Day Field trip, We need the following…

All Day  field trips require the permission slip/medical form, MHS/DSHS field trip request form, leave request for all chaperones that are MUSD staff.  The transportation and lunch forms only need to be filled out if you are utilizing  their services.

  • Complete REQ’s for all associated costs and submit on the CTE REQ submission form. Please include a flyer with cost listed or quote  

  • Fill out trip information on both the field trip request (MHS & DSHS)

  • Fill out  student permission/medical forms.(DSHS & MHS)

  • Fill out leave requests for Yourself and any other MUSD staff member attending the trip      *Include Activity registration info and Agenda  

  • Submit time off in Both OKTA and Frontline (Sub Request)  

  • If needed –   Fill out a transportation request and Email it to [email protected] Please include [email protected] on the email.  If you need a CTE Vehicle  please fill out the “CTE vehicle reservation form.”

  • Lunch requests need to be submitted 2 weeks prior to the field trip (if needed). 

  • Health Office Notifications
